Abstract
The Patient safety tool kit describes the practical steps and actions needed to build a comprehensive patient safety improvement programme in hospitals and other health facilities. It is intended to provide practical guidance to health care professionals in implementing such programmes, outlining a systematic approach to identifying the “what” and the “how” of patient safety.
