Key inclusion & exclusion criteria
Inclusion criteria: -15 years or older
-Documented attendence of at least 4 vitits during the previous year for hypertension or diabetes
-Having received treatment for these conditions

Exclusion criteria: -Patients who did not fulfil the inclusion critieria
-Patients who were unable to answer the questions in the questionnaere
-Patients who intended leaving the area during the coming year
