Key inclusion & exclusion criteria

Inclusion Criteria:

- Established primary care patient

- Spanish-speaking

- Depression (major depression, dysthymia, minor depression)

- Chronic medical illness (diabetes, hypertension, dyslipidemia, heart disease, lung
disease, cerebrovascular disease, arthritis)

Exclusion Criteria:

- Bipolar disorder

- Psychosis

- Cognitive impairment

- Active suicidal ideation
