Key inclusion & exclusion criteria

Inclusion criteria: Patients who are treated in neurorehabilitation, Phase B, C, D, aged 18-100Jahren
Exclusion criteria: Excluded are patients who themselves cannot give informed consent to the study and for whom no legalized authorized representative exists who could give informed consent instead.

Patients in neurorehabilitation Phase B, C, D

Intervention(s)
|
Group 1: Patients are assessed by a neurorehabilitation physician / a neurorehabilitation team as well as by a palliative care physician, not trained in neurorehabilitation using the surprise question ("Would you be surprised if this patient would die within the next 12 months?") and utilizing the palliative care basis assessment, HOPE-SP-CL. A telephone follow-up is planned to observe how far the assessment comes true and how patients’ health and living condition will develop.

Primary Outcome(s)
|
To test whether the surprise question is an appropriate measurement to predict the prognosis of neurorehabilitation patients. This will be checked by a telephone follow up after 12 months (are patients still alive or not?).

Secondary Outcome(s)
|
Are there any differences in the assessment of the surprise question between a neurorehabilitation physician and a palliative care physician not trained in neurorehabilitation? Which are the characteristics of neurorehabilitation patients who are still alive after one year or who have already died after one year, respectively? Was the care organized by the neurorehabilitation team for the time after patients’ hospital discharge appropriate, or not? A telephone follow-up (after 6, 12 months respectively) is planned to observe how far the assessment comes true and how patients’ health and living condition will develop.
