Extensive review of tuberculosis prevention, control and care in Kazakhstan

Abstract

The notification rate of TB cases in Kazakhstan is among the highest in the WHO European Region. WHO was requested to organize a review of the Kazakh Tuberculosis Programme, which took place from 10 to 16 May 2012. The reviewers made a number of recommendations to improve the programme. The main focus should be on: expansion of the programmatic management of drug-resistant TB, introduction and roll-out of molecular diagnosis of TB and MDR-TB, expansion of ambulatory treatment of patients, and home-based care and day-care treatment including all eligible patients, particularly sputum-smearnegative patients (as well as sputum-smear-negative MDR-TB patients). Hospitalization practices need to be rationalized in order: (i) to prevent the spread of drug resistance, and (ii) to use the available resources more efficiently by reallocating the funds saved by reducing the number of hospitals to expand ambulatory treatment, social support for patients and the number of health-care providers. Examples of the involvement of civil society organizations and provision of an integrated continuum of care (transfer of released patients from prison to civilian sector) need to be expanded across the country.

Implementation of the reaching every district [RED] approach: experience from North Sudan

Abstract

The purpose of this evaluation was to assess the extent and quality of implementing the Reaching Every District [RED] approach in North Sudan and its impact on immunization coverage. The evaluation was conducted in all 70 districts of North Sudan, excluding Darfur. District RED implementation data for 2006 were collected from district staff and used to quantify implementation by calculating Implementation Scores [IS] using a 10-point scale, with 10 being fully implemented. Overall RED IS ranged from 1.6 to 8.9. The percentage of districts with diphtheria-pertussis-tetanus [DPT] 3 coverage >/=80% increased as the overall RED IS increased, 78%, 87%, and 96% in low-, medium-, and high-scoring groups respectively. The degree of RED implementation varied across districts. Although it is not possible to directly attribute the overall increase in DPT3 coverage to RED implementation, RED implementation quality might be associated with improved DPT3 coverage

Abstract

The government of Saudi Arabia has given high priority to the development of health care services at all levels: primary, secondary and tertiary. As a consequence, the health of the Saudi population has greatly improved in recent decades. However, a number of issues pose challenges to the health care system, such a shortage of Saudi health professionals, the health ministry's multiple roles, limited financial resources, changing patterns of disease, high demand resulting from free services, an absence of a national crisis management policy, poor accessibility to some health care facilities, lack of a national health information system, and the underutilization of the potential of electronic health strategies. This paper reviews the historical development and current structure of the health care system in Saudi Arabia with particular emphasis on the public health sector and the opportunities and challenges confronting the Saudi health care system
