Abstract

En este informe un Comité de Expertos de la OMS analiza el tratamiento paliativo que necesita el paciente con cáncer y explora las causas por las cuales a menudo se le niega ese tratamiento. Además, se argumenta que el tratamiento paliativo, que tiene por objeto controlar el dolor, tratar otros síntomas físicos, y proveer apoyo psicosocial y espiritual, debe formar parte integral del tratamiento del cáncer. Se analiza asimismo la necesidad de promulgar leyes que permitan aumentar la disponibilidad de medicamentos analgésicos, la de ofrecer educación relacionada con el tratamiento paliativo, y la de establecer políticas nacionales que orienten la aplicación de programas de tratamiento paliativo. Entre los temas éticos analizados se incluyen el tratamiento destinado a prolongar la vida, la equidad en el uso de recursos limitados, y el derecho de todo paciente a un tratamiento eficaz de alivio del dolor. Se destaca que la forma más efectiva de mejorar la calidad de vida de los pacientes con cáncer consiste en aplicar los conocimientos ya existentes acerca del control del dolor y otros síntomas. El tema central de las numerosas recomendaciones del Comité es la necesidad de que se lleven a cabo esfuerzos concertados y coordinados a nivel nacional e internacional para promover la investigación, la educación y la concienciación del público en todos los aspectos del tratamiento del enfermo en fase terminal

Abstract

Regulatory problems in making opioid analgesics available and a series of policy and professional barriers often prevent proper treatment for people who suffer from severe pain. Over 40 experts from Bulgaria, Croatia, Hungary, Lithuania, Poland and Romania, along with experts from WHO and other organizations, attended the Workshop to evaluate national policies for opioid control and to develop action plans to improve the availability of these drugs for palliative care in their countries. The participants discussed the changes that might need to be made in laws and regulations or drug distribution to achieve this goal. They recognized the need to balance the regulatory requirements for control with the need to make opioids accessible for appropriate pain relief. The participants urged governments to ensure the accessibility of opioids for their population while complying with all international regulatory obligations

Abstract

Considers what can and should be done to comfort patients suffering from the distressing symptoms of advanced cancer. Prepared by nine renowned experts in oncology, neurology, pain management and nursing care, the book draws together the evidence and arguments needed to define clear lines of action, whether on the part of the medical and nursing professions or in the form of national legislation. Throughout, arguments for palliative care take their force from the magnitude of unrelieved suffering currently borne by the majority of terminally ill patients. Although methods for the relief of pain are emphasized, other physical, psychological, and spiritual needs for comfort are also included in the reports comprehensive recommendations. The concept of palliative care is explained in terms of its concern with quality of life and comfort before death, emphasis on the family as the unit of care, dependence on teamwork, and relationship to curative interventions. Subsequent sections concentrate on measures for the relief of pain and other physical symptoms, the psychosocial needs of the patient and family, and the need for spiritual comfort. A section devoted to ethics provides several important statements concerning the legal and ethical distinction between killing the pain and killing the patient, and the need to recognize the limits of medicine
